Assessments

To register for the Assessment process after completing your Initial Training you need to select the program for which you wish to be assessed in and complete the online payment process. Each assessment costs $45.00.

You need to download and complete the Assessment Form and attach this with a copy of your payment confirmation (automatically generated email to you once payment processed) to your VHS video/DVD.

Ensure you:

  1. Include the whole class inclusive of pre & post class interaction.

  2. View the class so you can make a self assessment.

  3. Send only on a VHS video or a DVD.

Please Note:

  1. If you wish your video to be returned please include a stamped self addressed envelope (with $5.75 postage affixed).

  2. Videos not returned will be kept for 20 days & then destroyed.

  3. Ensure that a sticker is attached to the video or DVD which clearly indicates both the instructors name and program being assessed.

  4. Our Assessment communications with you from this point on will be by email. Please make sure you have:

    • An email address, or arrange with your GFD to receive messages on your behalf

    • Checked that your internet email settings are configured to accept incoming mail messages from lesmillsusa.com
