You are being inconsistent, you said work out to your most extreme, rest and repeat, that requires time to get ones breath back. I suspect you mean just high intensity.
No I don't train with the underground as they are in a different country but I do similar level training where we do intense workouts stop briefly to run (yes run) to our drink bottles before the next track, music sounds like the chipmunks.
How often do I need to work out to maintain my fitness? Around 3-7 hours a day depending on the intensity. Have I suffered from sore knees etc, yes very badly, how did I fix, I stopped doing exercise that was high impact until my muscles were able to make them low impact. If you can't run on pavement without making noise then you need to work on your muscles so that each leg fully supports your weight. You can practise this while weighting in queues by standing on one foot. So when you run control your free foot so that it is placed on the ground not slammed on the ground. I found learning tango a great way of improving my core and helping me to learn how to walk properly.

Yes I did it for 2 hours, remember I am an endurance athlete. I sprinted as fast as I could up scree with a fully loaded back pack, if you have never done, it is like running up a very long escalator moving in the opposite direction. After sprinting I would take several minutes to get my breath back each time and I was feeling stuffed by the time I reached the top.

Mid intensity, high intensity are open to interpretation.
I have done low intensity = les mills, medium intensity = my current gym, high intensity = gym I go to in chch, and extreme intensity = other gym I go to in chch.
Tabata I have done a fair bit of, once did for 2 hours going up a scree slope. Personally I wouldn't recommend it for the unfit as it is better to do things slower and correct that faster and wreck your body. Before one can run one needs to learn how to walk properly.
Most of what I do is regarded as high intensity although is regarded by me as medium.
